So Josh interviewed Dr. Lofton and then I interviewed Dr. McCall. And we were just discussing this, but we basically asked you the same questions and interviewed you both for about an hour. And what you would have heard on the episode would have been less than half of what they actually said. Because we all know that doctors, you know, you could pretty much can talk down what they said and just take the meat away from it. No, but it was really interesting hearing, y'all basically said the same things, like hit on the same high points about about cardio, and we were asking you questions about those things, but we'll talk about that. We'll talk about that later. I kind of want to, what we didn't do on the podcast was like humanize you guys because in the business of media, you have to humanize people because are doctors people, Josh? 00:03:59 Speaker 2: Some of them are. 00:04:00 Speaker 1: Some of them are. 00:04:01 Speaker 2: Some of them are. Jason, you do everything. 00:04:10 Speaker 4: I mean, I don't do as much as I'd like to because of work. 00:04:15 Speaker 1: Well, what are your top three hobbies? 00:04:19 Speaker 4: So it has been gravel bike riding. And that's my wife and I started playing pickleball. And we kind of took. 00:04:27 Speaker 3: The new one. 00:04:27 Speaker 1: I've never heard pickleball before. 00:04:29 Speaker 4: But then I tore my calf muscle. 00:04:31 Speaker 2: He tore his calf muscle. He's in the total demographic of these middle-aged guys that get injured playing pickleball. 00:04:37 Speaker 1: You might be in my standup comedy skit. 00:04:40 Speaker 4: What do they call it? I mean, instead of tennis, it's basically tennis leg. I mean, pickleball leg. 00:04:45 Speaker 1: Pickleball. Okay. Let's stop on gravel biking. There's more. There's more. I know more of your hobbies, but. What's the difference between gravel biking and just mountain biking? 00:04:58 Speaker 4: So gravel biking generally involves more endurance, like long distance. There'll be a drop bar bike where mountain bikes usually have flat bar bikes, flat bar handlebars. And, you know, 20, 30 miles would be an average just quick ride. And I mean, up here, you'll see people doing, you know, 200 plus sometimes on there's, there's some. 00:05:18 Speaker 1: Riding like gravel dirt roads. So not like a mountain bike trail. 00:05:23 Speaker 4: No, you're riding gravel roads, which Southwest Arkansas is full of them. I mean, so I've ridden from Mena to Wicks with Randy and Randy's step, which he's, I think, been on here before. And then shout out to the long legged Randy step. So he's my riding partner. 00:05:36 Speaker 1: Okay. 00:05:37 Speaker 4: And I'm trying to get finished that up from Wicks to the queen, but yeah, all over Southwest Arkansas. And then I've been up here riding, but big tires, gravel roads, love being outdoorsy. And, you know, I've been chased by a hog. I was down at Pine Creek Bottoms, which is south of DeQueen, and hear a snort and look behind me and hogs coming after me. 00:05:55 Speaker 1: Really? 00:05:55 Speaker 4: Had to get after it, yeah. But, yeah, I just love being outdoors, you know, so you don't have to worry about cars as much. So, but, you know, big tires and long distance. 00:06:06 Speaker 1: Man, every time I see these bikers on paved roads, which is like every day, I just think, man, these guys are daredevil. 00:06:14 Speaker 4: Yeah, no doubt. 00:06:15 Speaker 1: These people are. Sometimes people think stuff that they might see hunters do would be dangerous. I think riding on pavement is dangerous. It's just me. 00:06:28 Speaker 4: People die doing that, unfortunately. 00:06:30 Speaker 3: Tom, you talked about that. We were going down. I don't know if you've ever been over to Logap. If you're coming down steel or coming down the mountain into Logap, We were doing in a race, and I bet we were doing 65, 70 miles an hour on these tiny 22-millimeter tires. And my buddy, I watched him cheat the curve a little bit, and somehow a car had gotten on the road at this segment, and it cheated the curve a little bit. And, I mean, there was an angel wing that was between them, and that's when I quit riding pavement. Oh, really? I sat up and said, my head, my hands, my kids, they're all more important than this little adrenaline rush that I get going down this hill. Wow. 00:07:12 Speaker 4: But I do like to send it going downhill. I mean, I've been 42 on a gravel road going downhill. That's my fastest, but. 00:07:18 Speaker 1: I bet that's fast. 00:07:19 Speaker 4: Yeah, I like the rush. 00:07:20 Speaker 1: Okay, so gravel bike and pickleball, which we're going to skip that. And he was set up trying to, I guess, really get his flavors out. And I remember seeing him there. And he's taking off. He cooks for Dallas Cowboys. How many green eggs do you have? Cooking on, like, say for cooking for a big group, depending on what, yeah. I mean, I grew up, my dad mainly fishing, but we, we go hunting and I, my kids are starting to get into it. Um, you know, Oakley's wanting to bear hunt this, this year. We'll see Silas too. So I've got to figure out how to get them on the stand at the same time safely and Yeah. But, yeah, I just love to do the outdoors, you know, anything outdoors. I mean, even my workouts. I've got some good leaders in my church. We spend a lot of time, if I'm not in the clinic, we're spending time together as a family. I've got good kids. I mean, I am a hunter. I play a little golf in between hunting season. Like as soon as spring turkey's up, I play a little golf. And then as soon as deer season kicks back in, I don't ever play golf. I think we talked about this last time. I did beat Lance Armstrong up a hill once. 00:12:35 Speaker 1: Oh, really? 00:12:36 Speaker 2: Uh-huh. 00:12:37 Speaker 3: Got a little article in Sports Illustrated because I did beat him up a hill. 00:12:42 Speaker 4: We talked about this when he was on the first time. 00:12:45 Speaker 1: Yeah. 00:12:45 Speaker 4: Yeah. 00:12:45 Speaker 2: Yeah. 00:12:45 Speaker 1: Okay. I remember. Yep. 00:12:47 Speaker 3: And so I used to bike all the time and then that happened. And, um, really since starting my own practice and owning a business that took, that became a hobby. 00:12:57 Speaker 1: Yeah. 00:12:57 Speaker 3: I mean, it's like a, it's a taking care of a lot of kids. 00:13:02 Speaker 1: Do you enjoy the business aspect of owning your own medical practice? 00:13:08 Speaker 3: That's a double-edged sword. Yes and no. I love learning new things. I love being able to do things differently. When you're employed, if you want to make a change, it's like turning a cruise ship. When you own your own business, you want to make a change, just make the change. It's not a big process. 00:13:28 Speaker 1: How much of being... Let me back up. I'm going to set the context and ask both of you a question. You feel like that someone who's a medical doctor, you just graduate from med school and you just like plug into the medical system, you help people, you make good money, and you're just, this is what you would think. And it just feels like, well, of course, but how hard is it to actually have a successful family practice, which is something both of you guys have built from the ground up as I understand it. So the question I guess also is how much of it is being a businessman and how much of it is being a, a medical doctor that just knows how to deal with people in their ailments. 00:14:22 Speaker 3: Yeah, no, I mean, I think that the, you don't know what you don't know until you realize you don't know it. And I think when you think, man, this is going to be easy. I've asked all the questions I've prepared for all the things. That's just the surface level. Um, I mean, I spend almost as much time running the business per week is what I do actually being a physician. 00:14:44 Speaker 1: Yeah. 00:14:45 Speaker 3: I mean, it's, uh, it's a, it's 80 hours a week and you get paid for 20. 00:14:49 Speaker 1: So you were trained in the medical field learning about humans, but really you got to be a business manager. 00:14:56 Speaker 2: Yeah. Yeah. 00:14:58 Speaker 1: I mean, is that what you signed up for? 00:14:59 Speaker 2: No. 00:15:00 Speaker 4: I tell, so I had a student from, um, Southwest Arkansas, his sophomore at SAU shadowed me this summer, and I told him he ought to go get a business degree with a minor in Spanish if you're going to go to med school because we don't get basically zero business experience or knowledge, and it's just learned by fire. The insurance industry makes it frustrating. It's frustrating for us as it is for the patient, and it's getting worse. 00:15:29 Speaker 3: We're doing something about that. 00:15:31 Speaker 1: Yeah, we're trying. 00:15:31 Speaker 4: Yeah. But it's not, that's, so I tell people, my wife helps, Cindy manages a restaurant called Stilwell's in DeQueen. It's like, you know, the transaction, you buy a burger or a sandwich and the transaction's done. Everybody gets, you get fed and they get paid. For us, well, the transaction's not necessarily done when the patient leaves and it might take months to get that. 00:15:51 Speaker 1: They buy the hamburger, but then- That person would have to go to the insurance company to get payment for the hamburger. 00:16:00 Speaker 4: We're waiting to get paid for that burger, so to speak, sometimes months and months and months because the insurance industry is trying to find out reasons not to pay us. 00:16:08 Speaker 3: Wow. 00:16:08 Speaker 4: A lot of times. 00:16:09 Speaker 1: The medical industry feels really stressful to me. 00:16:12 Speaker 2: Oh, yeah. 00:16:13 Speaker 1: It's broken. It honestly feels like people might just quit. We might just wake up one day and there are no doctors. 00:16:22 Speaker 4: It's starting to happen. 00:16:22 Speaker 1: Because it's so hard. 00:16:24 Speaker 4: Yeah, it is true. 00:16:25 Speaker 3: No, you're seeing, you're seeing doctors just quit. They're going, I'm not doing this anymore. It's not worth the stress. It's not worth the amount of work. And I think there's a, there's two sides of that. You know, you can go make money. If you want to make money, don't be a doctor. Yeah. You know, but I tell, I tell students all the time, you know, like I want to be a doctor. The, if you can't talk yourself out of doing it and there's nothing else that you could see yourself doing, that's a calling at that point. Man, go be a doctor. 00:16:53 Speaker 1: Yeah. 00:16:53 Speaker 3: Like the amount of difficulty is worth being able to do what, what the, what the Lord called you to do. 00:17:01 Speaker 2: Yeah. 00:17:02 Speaker 3: You know, and he gives you strength and, and the ability to navigate that. But man, if you could see yourself do anything else, go do that first. Like it ain't worth it. 00:17:12 Speaker 4: It's, it's, it's, it's hard. I mean, but it's doable, but it's hard. But again, it's, it's, it's a calling for me. I mean, it's, if it's, you're doing it for the money, you will burn out. 00:17:20 Speaker 1: It's not, you think most people would, would, say that about their profession in the medical, well, as doctors, that they feel called to it? 00:17:28 Speaker 4: I think initially, yeah. I think a lot of us want to help people. We really do. I think most people are very altruistic and want to help people. Now, do some get skewed by the money or, you know, jaded? Yeah. I mean, it's the system. Like, we all want to spend more time with patients, but we can't in the current insurance industry system because that doesn't pay the bills. So you have to see so many to get the bills paid. And that's where patients want to spend more time with doctors. We want to spend more time with patients, but from a business standpoint, it doesn't add up at some point if you slow down too much. 00:18:04 Speaker 1: Interesting. 00:18:04 Speaker 4: But, but yeah. 00:18:05 Speaker 1: So how do you, how do you, how do you fix that? 00:18:09 Speaker 3: Man, I think the, the main major issue is going, there's no transparency in the medical industry and the, especially on the payer side. And so it's, how do we reduce the opaqueness of, to really see what goes on so that we can reduce the, reduce the over complex system to make it simple. You know, you, you see me, you pay me for my advice, whether you take that advice, that's on you, but you paid me for it. And that transaction is done, you know, and I agree to take care of you and answer your phone calls and stuff like that. Um, but this whole issue of, of paying $ 2, 500 a month for your family to, you know, an insurance company, And then you've got to pay $ 10, 000 more and you don't know how much stuff costs whenever you get it done. That has to go. Because what it does is it reduces people's confidence to enter the medical system when something's wrong. Right? Like we talked about that yesterday. And so my goal or the next, however long it takes, is to reduce the complexity and introduce just the simpleness of it. Because, I mean, there's so much beauty in a relationship with a patient and a physician together. that when they know each other, man, you can change the world with that. At least you can change somebody's health, which, man, it's a big deal. 00:19:34 Speaker 1: Well, I commend you guys for just the service of what you do. So, I mean, there's plenty of occupations that people are providing like a real service to people. I think about teachers. I think about first responders. But really, doctors, I mean, fit into that. 00:19:56 Speaker 4: You give up a lot of your life. Yeah. to get to just a square one of where you start being a doctor and working. 00:20:02 Speaker 1: How old were you when you became a medical doctor and just graduated school, I guess? 00:20:07 Speaker 4: So I graduated residency in 2007, so I turned 31 in September. We graduated in June of that year, so basically 31 years old. 00:20:17 Speaker 1: Were you going to school that whole time, or did you have a couple of starts? 00:20:19 Speaker 4: I had one gap. So I graduated University of Arkansas in 1999 and took a gap year. I was on staff with the campus ministry for a year. and then started med school. 00:20:30 Speaker 1: It took that long. 00:20:31 Speaker 4: So it's four years of med school, three years of residency, and four years. So it's 11 years total just to become a family physician right now. 00:20:37 Speaker 2: Wow. 00:20:38 Speaker 4: That's a long time. 00:20:38 Speaker 3: How old were you? 29. 00:20:41 Speaker 1: So it takes that long. 00:20:42 Speaker 3: Yeah. 00:20:43 Speaker 4: But stuff like, example, I was studying in Little Rock one Saturday night, and George Strait was at War Memorial Stadium in a concert, and I couldn't go to it because I had a test. Now, hey, there's some guys out there that they could have gone to the concert and still made an A. I was one of those guys I had to bust it and gave up a lot. 00:21:01 Speaker 1: What if it had been the turnpike troubadours? 00:21:04 Speaker 4: They've been tough, you know, I mean, you know, maybe I could have studied, you know, I went, I went to school and I said, well, I'm going to give up God or exercise. And that, and I didn't. And those are two things that I stayed committed to have to this day. 00:21:15 Speaker 2: So. 00:21:16 Speaker 3: Wow. 00:21:16 Speaker 1: That's good. That's good. Okay. I've got to tell you the story that Misty told me. 00:21:22 Speaker 4: Okay. 00:21:23 Speaker 1: I was on the phone the other day with a guy, Misty's in the car with me. And, It's a guy I don't know, but it's just a transaction. I've got to pay him some money. And he goes, yeah, I'm from Okemo, Oklahoma. What does Clay say, Josh, if someone tells me they're from Okemo? What would I say? 00:21:44 Speaker 2: Do you know Evan Felker? Bam. 00:21:47 Speaker 1: Man, you know what? I know a guy from Okemo, Oklahoma, Evan Felker. And the guy goes, oh, yeah, I knew Evan since he was that tall. 00:21:56 Speaker 3: You know. 00:21:57 Speaker 1: And we get off the phone and Misty's dead serious. She does not think she's going to be on the podcast. And she goes, Clay, do you think you could go one month without telling somebody that, you know, Evan felt she was not trying to be cute. And I just said, Misty, he told me he was from Okima. I had no choice. And she said, yep, that was kind of an exception. You tell– she thinks that I just walk into, like, a store and I'm just, like, looking to tell someone that I know Evan Felker. 00:22:34 Speaker 4: That's funny. 00:22:34 Speaker 1: And so every time that I even say– I mean, I don't know. Turnpike Troubadours comes up every now and then, okay? It just does. We talked about men's health. You know, that was me. Like, whatever you need signed, I'll sign it, you know. But I didn't. I kept it quiet. And then my buddy next to me, Mark, he goes, hey, he was the guy. He was the doctor that was on that render. I cited this study that I, that I saw online about how they had studied muscle mass with even people in their seventies, eighties, even into their nineties. And they were talking about like a hundred percent increase in, Like the weight they could lift, which, I mean, if they could lift two pounds and then they could lift four pounds, like that would be, you know, double. So obviously the gains would be smaller if you were that old. But the point is that it's never too late to start doing something that is going to impact your health. And that's encouraging because I think sometimes people just think, man, that ship has sailed. 00:26:32 Speaker 3: I mean, you take that even and you make it practical, two pounds to four pounds is, someone being able to pick up a half a gallon of milk and import in their cereal. I mean, it's things like that that you don't really even think about that would make a meaningful change in someone's life. 00:26:46 Speaker 1: That's true. Yeah. And I also didn't want people to think that this was a podcast for, like, old people. 00:26:54 Speaker 2: No. 00:26:55 Speaker 1: I said it right at the beginning. I was like, hey, this is not– I mean, there's some applicable things, really, for people probably in their middle age, but I wanted bare– to listen to this and be like man if i start building now healthy eating habits healthy exercise building muscle i mean you can just have such a major runway when you hit you know the middle of your life and on that it could be really major. 00:27:22 Speaker 4: Yeah my biggest regret is on josh is that i didn't i just started weight training really in the last two years i've always ran always rode bikes and you know it's good for my heart but it just hadn't built muscle and But I'm enjoying it now. And it's not bodybuilding. I'm just trying to do functional fitness kind of weight training, meaning can I get off the ground? Can I lift this or that? And I wish I, in my 20s, kept that going. 00:27:47 Speaker 3: Where did you pick? I enjoyed the clip on farmer's carry. Tell me about that because I've never. 00:27:54 Speaker 4: So, you know, like if you're on a farm carrying buckets, they just have the name of farmer's carry. So I have kettlebells. I work out with kettlebells. It's almost exclusively. And I've got 44, you know, it's 20 count, 20 kilogram, but 44 pounds sounds better. 00:28:08 Speaker 1: Sounds like twice as much to me. 00:28:12 Speaker 4: Yeah, 44-pound kettlebells, and I do 100 yards down, rest, 100 yards back. And I can go down and back the first rep, but the second one, my arms start burning, and I have to usually set it down about the 40, 30-yard line. 00:28:25 Speaker 1: I started seeing people do that and learned that it was called a farmer's carry. And at first, I didn't think much about it. I was like, what's this doing like it feels like when you isolate a muscle group and like do a bench press or leg press you like feel the burn but i'm big on farmer's carries it it when you really stress yourself out carrying that weight it feels like you're working every muscle in your body yeah that's that's it just it's. 00:28:52 Speaker 3: Just it's functional too yeah like how many times are you going to be stuck on a bench press right trying to lift something. 00:28:58 Speaker 1: With a rock on your chest yeah exactly. 00:29:01 Speaker 3: Like Man, work out to the way your lifestyle is. 00:29:04 Speaker 2: Yeah. 00:29:04 Speaker 1: Yeah. 00:29:05 Speaker 4: So everything I do is all about like my, you know, first it's called a Turkish get up. You literally start on the ground on your back. You lift the kettlebell with one arm all the way up and you go back down. So, I mean, that's training my body to constantly go up. 00:29:19 Speaker 2: All the way to standing? 00:29:20 Speaker 4: All the way to standing over your head and then you come back down and you do both sides. And so it's as much of a whole body, you know, workout as possible. And so that's just one of my. things I do inside of that. So those are those functional, I'm looking online constantly trying to figure out what's something functional I can do to add to what I'm doing. 00:29:39 Speaker 1: Is the, is the whole strength training, adding muscle idea for longevity. I know it's not new, but it feels like it's new on the mainstream platform of going. 00:29:52 Speaker 3: Out last five years. It's really, it's found, it's found its ground. Yeah. 00:29:57 Speaker 4: And just the data, you know, it's because now we have research and data showing what this does beyond the cardio, because the cardio was the focus, it seems like, forever. I don't ever remember hearing strength training and emphasis in med school. 00:30:08 Speaker 3: No. 00:30:08 Speaker 1: It's such a no-brainer, though. It is. I mean, when you, because when I heard it, I heard it was a lady doctor that was on Huberman. Gabriela, real famous kind of influencer person. Anyway, I heard her talk about your muscles are the organ of longevity, I think is what she said. And it just clicked. It was like, well, I guess that makes sense. And then now everybody's talking about it. 00:30:39 Speaker 3: Yeah. 00:30:40 Speaker 4: And Adam mentioned, Dr. McCall mentioned, you have to have your heart to get you there. But if your muscles aren't going to get you out of the chair or off the ground, if you fall, then you lose your independence. And that's what we, you know, your muscles are what are going to cause you to lose your independence. 00:30:54 Speaker 2: Mm-hmm. 00:30:55 Speaker 3: I think that's the difference between a 42, 43-year-old and a 50-year-old. I was talking about still going hunting. Jason was talking about maintaining the independence. Josh has got me beat by a few months. 00:31:11 Speaker 2: I got him beat by four or five months. 00:31:13 Speaker 4: But, you know, I do, like Clay mentioned, I want to be hunting at 70 if I can or rafting with my grandkids. You know, I want to be able to do those things. And, you know, there's some data that says, hey, we don't necessarily– We might not peak at 40 or 50. If we keep at it, we might can actually peak out even in like your sprint training. I saw something saying, hey, at 60 or 70, you can still hit those times that you were hitting when you were younger with proper training. 00:31:34 Speaker 2: Wow. 00:31:34 Speaker 1: Well, I said this in our interview, Dr. McCall, but it didn't make the cut. About when I turned 40, I felt myself getting weaker, just physical activity. And I, like you, cardio trained kind of my whole life just physically. ran, stayed active, like at any time between 15 and 40, I could have got out of the chair and ran five miles, but never weight trained other than pushups, which are questionable. Um, and, but when I was about 40, I just noticed I just wasn't as strong as I was, you know, throwing a saddle up on a mule or picking up a bag of dog food. And, um, But I looked the same. I felt like I weighed the same. My belly wasn't getting any bigger necessarily. And I was kind of like, this is not good. And then that's about the time, five years ago, that everybody was like, oh, you got to add muscle. And I'll be brief. When you turn in your mid-40s, you realize some of the things that you said when you were younger were just dumb. And this is one of the. 00:32:50 Speaker 2: Things most of what I said when I was, well. 00:32:54 Speaker 1: I am really trying to work on having less of, of these just like very opinionated statements that just aren't true. I used to say that I was philosophically opposed to public working out. Okay. 00:33:10 Speaker 2: Okay. 00:33:10 Speaker 1: It's just kind of an insecure, dumb thing to say. And I just kind of thought it was funny, but it was actually true. Like if you would have, even six years, five years ago, been like, Clay, you're going to join a gym and you're going to go there a lot. 00:33:26 Speaker 3: Yeah. 00:33:26 Speaker 1: I just would have said that will never happen. Now I, and then my, my youngest son started, he's a ball player and was going to the gym here in town and kind of in an effort to just spend a little more time with him. I was like, you know what? This was three going on four years ago. I was like, I guess I'll go up there. And I was, so insecure about it i really was. 00:33:51 Speaker 2: Yeah i just it can be really intimidating. 00:33:53 Speaker 1: Yes it can i just i just didn't like working out just around all these people that i didn't know and uh i just kind of pushed through it and just did very minimal stuff but just kind of worked some of the major muscle groups out and i was like oh this can't hurt and basically just started building on top of that and after about two months of going to the gym, you realize that nobody cares. Nobody's watching you. 00:34:21 Speaker 3: Right. 00:34:22 Speaker 1: And today I just go in there and do my stuff and I don't think a thing about it. 00:34:28 Speaker 3: Yeah. 00:34:28 Speaker 1: And it's, it truly has changed my life and I look the same. That's, that's what's, um, I think if I'd worked out this hard when I was 25, I might've been, you know, you might've seen more of a visual impact. Uh, But I truly am in some of the best shape of my life. I'm strong as I've ever been. And I really think it has altered the course of my life. 00:34:58 Speaker 3: No doubt. I mean, that's what we talked about. I mean, you put a ton of deposits into your bank account. 00:35:03 Speaker 2: Yeah. 00:35:03 Speaker 3: I did like how we– like that was– we both talked about that. Like if you're investing, the dividends will pay off. 00:35:11 Speaker 4: You know, I tell people– Good analogy. My patients like– You know, we call 40 over the hill, right? So if you think of 40 being like a hill of strength, you're starting to go downhill after 40. And if you're not strength training, it's like if I use a bicycle, for example, if you're on a bike, you're going downhill with no brakes. And when you start strength training, all of a sudden you add brakes to that bike and you're not going to stop the descent, but you're slowing the descent down so that at the bottom of that hill, you've still got that independence. Without that, it's just however Mother Nature takes you down. 00:35:40 Speaker 1: Yeah. 00:35:40 Speaker 4: So you can add that brake at any time, any point. GLP-1. GLP-1. Can either one of y'all talk about those and kind of the controversy? Give me the controversy that people might think about it. And that's probably not the right word, but. Pros and cons. 00:36:14 Speaker 3: Yeah. 00:36:15 Speaker 1: What are they? 00:36:16 Speaker 3: So GLP-1s are. 00:36:18 Speaker 4: Glucagon-like peptide. 00:36:20 Speaker 3: Yep. And they're not always just GLP. There's a couple of ones that are GLP and GIP, which is gattric inhibitory peptide, glucagon inhibitory peptide. And they're basically these... You think of a peptide as being a form and at a job. You can have all the people there. You can have the workers. You can have the surveyors. But the peptide is the foreman who shows up and says, you go do this, you go do this. And basically that is what it starts to signal. And the way that the GLPs work is they slow everything down, they stabilize your insulin, and then they tell your brain that there's no dopamine. Like a lot of times you'll eat something, you'll get a big dopamine surge from that. And it tells your brain, that wasn't very good. You didn't get enough dopamine. So why are we eating that? And it really is a game changer for most people. 00:37:22 Speaker 1: People are using it to lose weight. 00:37:24 Speaker 3: Yeah, absolutely. 00:37:24 Speaker 1: The GLP-1. 00:37:27 Speaker 3: And I think the controversy is people say, well, we don't know what it's going to do in 20 years. Actually, we do. You're going to lose weight. It's been used for 20 years. 00:37:36 Speaker 2: Yeah, by Etta. 00:37:39 Speaker 3: Came out in 2001, and we've been prescribing that. 00:37:42 Speaker 2: And it's primarily been used for people with diabetes. 00:37:47 Speaker 1: What are the names of this? 00:37:48 Speaker 3: Is this Wegovi, Ozempic? 00:37:52 Speaker 4: Zetbound, Manjaro. So you've got two main ones, Terzepatide and Semaglutide, which are the generic name. They're not generic yet, but the base name. But Manjaro is Terzepatide, and Ozempic is Semaglutide. 00:38:07 Speaker 1: And so people are using... these drugs pretty much to lose weight. 00:38:12 Speaker 4: Diabetes no but like say the lady at church recently got on one and she came two weeks later said my arthritis is almost completely gone and so there's other impacts that it's having besides just diabetes and weight loss. 00:38:26 Speaker 3: We know the glp1s have about they're 70 active in reducing inflammation yeah so i've seen people who had crones go into remission I've seen people who had RA go into remission just from GLPs. 00:38:45 Speaker 1: So you guys, both of you are really positive on these? 00:38:50 Speaker 4: I tell people it's as close to what I would call a miracle drug as anything we've seen in 20 years that I've been a physician. 00:38:55 Speaker 2: Really? Yeah. 00:38:56 Speaker 1: Why are people so, when you click on social media, you hear all these people saying, oh, this is, beware of this. 00:39:05 Speaker 4: You know, well, one, I would say that some people call it the Ozempic face because they're losing lots of weight and your skin stretches and it doesn't, it's not elastic and stretching back. So, you know, they have that look. But another thing is, you know, I tell all of my patients, I think too many people are on these that aren't doing it, but if you're not strength training while you're taking these, 30% of your weight loss is muscle. So for every 10 pounds somebody loses on this three pounds of muscle if they're not strength. 00:39:32 Speaker 1: Training okay that brings up josh because i have heard that that's that's one of the top things people are like yeah these glp ones work but you lose a bunch of muscle mass and then we know that muscle mass is the is the Pathway to longevity. So really, you know, you're kind of paying, cheating. What's the phrase? Cheating Paul? 00:39:56 Speaker 2: Robin Peter. 00:39:56 Speaker 4: Pay Paul. Yes. 00:39:59 Speaker 1: Yeah. But you're saying if you strength train while you're taking this, it's going to help. 00:40:05 Speaker 3: It's the cheat code. 00:40:07 Speaker 4: Yeah. And honestly, that's any diet. If you're just losing weight with diet alone, you still need to strength train to keep from losing muscle. Okay. 00:40:15 Speaker 1: Okay. Now that's good intel. 00:40:17 Speaker 2: Yeah. 00:40:18 Speaker 1: Because if you just reduced your calories to 700 calories a day and didn't do any strength training, your body would eat some of your muscle. I mean, that was where you would lose some of the weight? 00:40:29 Speaker 3: Absolutely. 00:40:31 Speaker 1: Okay. So, Josh, I actually asked Josh before I put on the podcast what I did. I said that because he quickly gave a synopsis that he'd lost 50 pounds and And then Dr. McCall had talked about just how just this radical transformation. I mean, even inside your labs, your blood work, your, all your stuff, just kind of like with that weight, weight drop loss and a change in lifestyle. 00:41:00 Speaker 3: Right. 00:41:01 Speaker 1: So big stuff happened. Yeah. 00:41:02 Speaker 2: So the one I met with, with Dr. McCall was, it was December 30th. And so we kind of made all these changes right at the beginning of the year. And, and there were, there were certain things that, you know, it the primary thing was losing weight. Like you've got to lose weight. I was, I was 271 pounds and which is way more. 00:41:22 Speaker 1: Than your five, six. So that's six foot one. 00:41:26 Speaker 2: I'm six foot one, but it's still, it's way too much weight for my body. I'm a fairly big, big framed guy, but still 271 pounds is way more than, than I need to be carrying. And, uh, you know, he talked about, and I'll, I'll tell you, I had, I had a lot of other things that, that were, ailing me as well. I had, I have two, two messed up rotator cuffs from some skiing accidents. One, one has a partial tear into tear. And, uh, I was having to get, um, cortisone shots on my shoulders about every six months just to manage the pain. And, uh, he suggested, um, he suggested a GLP and I, I didn't know a lot about it. And so the great thing about, about talking with dr mccall is he he takes all the time that you need to communicate and to allows you to ask questions so i mean i've just bombarded him with questions and uh that was one of the things and i appreciated the fact that you know he when he said hey let's look at your health you know he was looking at my heart he was looking at my weight he was looking at my sleep he was looking at my testosterone he was looking he you know he asked me about my spiritual life i mean just Like he said, we need to look at this holistically. We can't just treat one thing and expect it to change your life. And so it really was a decision and a recipe to benefit everything. But losing weight was the thing that we needed to do that would have the most impact on my health system. Because my blood work was really bad. I had high cholesterol issues. low testosterone and, you know, I've been pretty transparent about, about, you know, having can't testicular cancer. Um, you know, that, that caused, it was a big part of why my testosterone was down, you know, lack of exercise. And so, um, in doing that, like we really talked about the need for, I think anything that you rely on as a, as a crutch or a cure to fix you, um, will be successful for a time. And then as soon as that wears off, you're going to, you're going to fail. And so I knew that, that getting on the GLP, you know, we talked and he said, Oh, you'll, you'll probably lose a lot of weight. I didn't, I didn't know how much I would lose, but, but he said, you'll, you'll lose a lot of weight. But I knew that I couldn't just rely on that to fix it. I knew I had to change the way I was eating. And I knew I had to, I knew, I mean, I'm, I'm 50, I just turned 50 in May. And I was noticing how much muscle, you know, I've always been a big guy. I can't say as I've ever been real fit, but I've always been strong. But I had noticed how weak I'd gotten over the last five to seven years. And so I immediately changed my diet, started focusing real heavily on protein and fiber, and then started the GLP and started a really low. 00:44:33 Speaker 3: Dose of How did you take it? 00:44:35 Speaker 1: Is it a shot? 00:44:35 Speaker 2: It's a shot. It's an injection you do once a week. 00:44:38 Speaker 1: You do it yourself? 00:44:39 Speaker 2: I do it myself. Yep. Yeah. I, I, mine is compounded. So the pharmacy sends me a vial and I just draw it up in a syringe, syringe once a week. I mean, it's like taking insulin. You, you stick yourself in the belly and a tiny, tiny, tiny, you honestly can't even feel it. 00:44:56 Speaker 1: And, uh, do you work for big farm? 00:45:00 Speaker 2: No, I do not. Um, But then I started alternating running and weight training. And when I say weight training, I mean like really emphasizing heavy weight because I know that heavy weight is going to build my muscle. It's also going to increase my testosterone. And so I really started focusing on heavy weight and getting strong. But I know that my heart's not super fit. So I wanted to be able to do aerobic exercise to try to get that VO2 max. I'm sure my VO2 max when I started was in the dumper. So I've really been focusing on that. And I honestly have been working out six to seven days a week. And I don't always necessarily work out for an hour, an hour and a half. There are days where I work out, I'll weight train for 45 minutes to an hour. And there's days where I weight train and run. 00:45:55 Speaker 3: In a day. 00:45:56 Speaker 2: But I'll just go, I'll go run three miles. And it now I'm down to, it takes me about 28 minutes, 29 minutes to run, to run three miles. And, uh, I, I tell you, I think I'm probably like more functionally strong now than maybe I've ever been. And I feel like I'm more fit than I've ever been. And that's, you know, that's just in the last eight months, but I've been really consistent with it for eight months. And so I, you know, I've dropped 50 pounds. Um, My, my labs, I mentioned this on the podcast, my labs on Adam, he, he actually was coming here for the podcast. And I said, did you see my labs? And he goes, no. And he pulled him up on his phone and he goes, Josh, this is not good. Like I saw the look on his face and I was like, oh crap. Like he's serious about this. And, uh, one of the first things he said was, I think you need to see a cardiologist. And that was like, oh my gosh. I need to see a cardiologist. You're too young for that. I can't believe I'm at this point. 00:47:02 Speaker 1: Right, right. 00:47:05 Speaker 2: And that was one of those things where it's like, I got to do something or I'm going to be having a heart attack. And so just in about four to five months, my labs went, my testosterone doubled. 00:47:20 Speaker 3: My labs, my. 00:47:22 Speaker 2: Lipid panel, which is your heart, your cholesterol and all that kind of stuff, went from off the charts to down at the lower end of normal. And so, I mean, just in eating better and exercising and losing weight, it's been seriously life-changing. And so I would encourage anybody out there who thinks, and I had gotten to the point where I wasn't doing much physical. I mean, I was, I'm out, I'm outdoors and I'm hiking and that kind of stuff. But as far as intentional, um, exercise wasn't doing a whole lot, um, that you can do it. I mean, you just, you have to, you have to make that initial thing. I will tell you that momentum is such a valuable thing. Once you get that ball rolling, you want to keep it rolling. Um, You know, I can't say that changing my diet makes me feel a whole lot. Like, I don't notice this huge change. I've got so much more energy. But the combination of the exercise and the diet and the losing weight has given me a ton more energy. And I feel so much better. I feel better than I've felt in 20 years, probably. So, anyway, I would encourage, I mean, if you're struggling with your weight and you've dieted, and lost weight and put it back on i mean i've been through that cycle a number of times if you're if you're that's you talk to your doctor about a glp i mean i'm not a i'm not the kind of guy that's just like hey just go take a drug you know what i mean but but you can't just rely on that thing to fix the problem because really the problem is inside of you you know what i mean you have to you you you have to purpose to do the right thing you have to purpose to make the right choice but something like a GLP can help. It can help with that. And so can't rely on it, but it can help. So man, I encourage anybody out there. Um, I also encourage you if you haven't had, if you haven't been to the doctor, if you haven't had your blood work done, I had no idea that my, my, I was in that bad of shape. I mean, I knew it was heavy. Right. But I had no clue what the impact was. And so, do it. 00:49:50 Speaker 3: Yep. 00:49:51 Speaker 4: And most insurances pay for once a year visit. Or is it all about their behavior along with taking the drug? 00:50:24 Speaker 3: I mean, I think both. But, I mean, I have one of my patients. We've tried everything. I mean, he's kept calories. Within a couple, like I had gotten to the point where I thought I was going to have to have rotator cuff surgery on my right side because it was so painful. I mean, it's not easy to, I mean, we, you know, there's a saying you can't, can't out exercise a bad diet because you can be exercising, but if you don't change your diet, you're not going to see those results. But, but, you know, I, I tell people I could foresee this being almost like a vitamin in the future. If we can get people on it, we can prevent some of the diabetes, the knee replacement, just from the weight. So for every pound someone gains, they add four pounds of pressure to their knee, six pounds to their hip. If you can lose a pound, you're taking four pounds of pressure off your knee, six pounds off your hip. So just think of the joint replacement since it's weight-based. But diabetes, heart disease... If we see somebody hitting that trajectory right now, it's hard to get people on. The tricky part is the cost and getting it approved by insurance. Because I'm a preventative-based mindset. The problem with medicine is... it doesn't pay for prevention. We know that. But one thing that you can do with it is microdose it. And it helps you change how you eat because the food that you did eat doesn't taste the same. It doesn't taste good anymore. I don't think it was in the podcast, but I've used it to help people quit drinking alcohol. Most days I eat once a day. And you would think I would just drop pounds, and I haven't. And the skinniest I've ever been was when I was training for, like, hard half marathon stuff. Give yourself some on the weekend. It's okay. Like that, that can be a diet. And there are some good doctors over there. There's some smart guys. You've got to be careful. It's kind of like if you go and get water out of a pipe in the ground, it might be great. clean water, but it could be also contaminated with Giardia, and it looks the same. And so we've got to know what our source is. And so there's a lot of peptides. So some can be taken daily. Some can be taken... Some have to be cycled. 00:59:55 Speaker 2: So. 00:59:56 Speaker 4: Like some of these peptides increase blood flow to maybe certain areas, but if you've got a cancer that you don't know about, you don't want to increase blood flow to your cancer. And you mentioned compounded. They also put in either B6 or B12, which can help with some of the side effects and the cost of it. It's so much more reasonable for people And you go, man, if it works, why would I withhold something that was, you know, I think what we talked about, if you come to see me, my whole thing is I'm looking at your numbers, not because they're a number, because I go, what is that going to steal from your life? Like, man, did I want Josh to lose weight because his BMI was high? Like I didn't look at the BMI and go, man, that's too high. Let's get to that goal. I said, You're going to be able to do more. Your knees are going to feel better. And you're going to be able to live more life that God has intended you to live. I heard a guy one time just say, He said a human is designed to be out of breath every day of their life, which I thought was a pretty good just way to think about it. Get your heart beating every day. 01:02:58 Speaker 4: That's something that I've tried to implement is adding kind of some sprint training to what I do, whether it be on a bicycle or just at the football track where I've been running. It's something to get your heart rate up at a high zone. We've got five heart rate zones, I guess, one through five zone, one being your just casual zone two is considered that aerobic kind of threshold. You want to stay in, but then zone three, four and five are, are zones that you start hitting your anaerobic threshold. But we need, we don't hit that enough. You know, like our ancestors were chasing or being chased and we don't do that enough anymore. 01:03:33 Speaker 2: Yeah. 01:03:34 Speaker 1: And then the third thing being your diet. Yeah. And I'm sure there's something, there's things that I eat that are my hot Cheetos that we all have, but, but, but the American diet is just pretty messed up. And, and, What I was trying to focus on, and it's because it's been an emphasis inside of my life, is not letting the culture dictate what I think is normal. Because I think we do that. I think we see, we compare ourselves to the rest of the world, and it is not very hard to be fitter than the average American. And I mean, you could be more fit than the average American and still be pretty bad out of shape. And then, you know, I said, and if you, I think if you analyzed the people listening and, you know, if you analyze your world, you might find this to be true. Maybe you've thought it before, but just how casual we are with like being bad out of shape. I mean, guys laugh about it. Oh, yeah. Here I am talking about comparing yourself to other people. On the other side of it, you know, there's a lot. There is a lot of genetic variation in people. And you may put a bunch of inputs in and never get what someone else gets. But you got to erase that and find what's healthy for you. Um, I don't want to be the guy that like, like, you know, talks about like people dog, cam Haynes all the time, more power to cam Haynes, man.
